Care Coordinators Needed on Maui! Imua Family Services is looking for Care Coordinators to become a part of our team. This position will provide care coordination and management for families of children 0-8 with developmental delays. Imua Family Services is a non-profit serving Maui, Molokai, and Lanai since 1947. Minimum Qualifications Bachelor's Degree from an accredited college/university required. Pediatric experience with birth to eight populations is preferred. Salary starting at $41,000/annually, Commensurate with Experience IMUA Offers: Comprehensive salary Medical / Dental / Prescription / Vision Paid Time Off, Accrual begins immediately 19 Paid Holidays (Including Christmas through New Year's) 401k with company match after one year of service This position is responsible for the provision of family-centered culturally competent care coordination and social work services for children and their families, birth to age eight with typical development and developmental delays. This position provides interim and on-going care coordination services to families receiving services from Imua Family Services. The individual compiles information and the data necessary for programming and for accurate reports and billing to the Department of Health, Medicare medical insurance, and various funding sources. Essential duties and responsibilities also include providing information to other professionals and the community as well as conducting other clerical work and administrative duties as required. www.imuafamilyservices.org
