aQua Dialysis Clear Lake is seeking a full time Home Therapies RN t o provide excellent quality care for our dialysis patients . An ideal candidate has at least a year of nursing experience and at least six months of experience with peritoneal dialysis. This position includes PD and home hemo. The Home Dialysis Nurse trains the patient and/or care partner to successfully self-administer dialysis treatment . The nurse also c onduct s home visits to assess the patient's home environment and evaluat e patient outcomes . They obtain data to track program and patient performance to improve quality of care . This position is primarily Monday through Friday and includes on-call nursing services , nights and weekends, on a rotational basis as needed or assigned. They may have to be f lexible with hours to accommodate patient s .
