The position would be responsible for ensuring delivery of high quality care practices in the treatment of symptoms and promoting the highest quality of life for homebound patients and their families.#INDNURSESResponsibilities:Supports organization's mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workersAssesses, diagnoses and develops appropriate plan of care to treat homebound patientsMonitors the patient's condition and responsiveness to the treatment providedActs as an agency representative with patients and their families as well as with other health agencies, groups and organizationsAdheres to agency infection control and safety policiesAdheres to the organization's policy in regards to absenteeism and appearanceOmission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this positionQualifications:Master's degree in Nursing required from an NLN accredited programLicensure as an Advanced Practice Nurse (APN) in NJ with prescriptive authority; maintain a CDS and DEANational certification as an Adult or Geriatric Nurse Practitioner2+ years of APN experience and/or significant experience as a registered nurseExcellent communication, organizational and interpersonal skillsStrong computer skillsWorking Conditions/Physical Demand: Working indoors/outdoors with Client/Customer contact and clinical equipment and use of phone and computer; valid driver's license and automobile insuranceEEOC: VNA Health Group provides equal employment opportunities to all employees and applicants for employment and prohibits discrimination and harassment of any type without regard to race, color, religion, age, sex, national origin, disability status, genetics, protected veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state or local laws.
